1. Make sure your information on this form is correct
  2. If returning multiple returns, make sure you have a return form for each item enclosed in the box
  3. Enclose all product and returns forms in the box
    • If you have your original invoice include a copy to help in process time

    • If you have your original invoice, you can use the prepaid peel off label from the front of your invoice.
    • If shipping back using your own carrier, we recommend using a trackable shipping method.
  5. Please expect your return to take 3 – 12 business days for processing from the date of receipt at our returns department.
  6. We will send an email confirmation to you once your return has been processed.
  7. If your replacement product is not available, a site credit will be emailed to you that you can use on our site or through our call center.
