1. Make sure your information on this form is correct.
  2. If returning multiple returns, make sure you have a return form for each item enclosed in the box.
  3. Enclose all product and returns forms in the box.
  4. Mail your return back to:

    If shipping back using your own carrier, we recommend using a trackable shipping method.

  5. Please expect your return to take 3 – 5 business days for processing from the date of receipt at our returns department.
  6. We will send an email confirmation to you once your return has been processed.
  7. If your replacement product is not available, a site credit will be emailed to you that you can use on our site or through our call center.
  8. For refunds, you will be issued a site credit by email which you can use online or through our call center.
  9. If you do not provide a valid email address, we will be unable to issue your site credit.
  10. All site credits will expire after 1 year of issuance.
